APPOINTMENTS for New Patients

As we are currently extremely busy, and booked well in advance, Dr Ward will need to see your referral prior to making an appointment date for you. Please fax referrals to 9386 5648, attach referral to an e-mail (use the link below) or post to the Hollywood Office. Dr Ward will then be able to prioritise cases and allocate an appropriate date. Our staff will call you subsequently (may be a day or two later) with a date and time.

You can speak with our reception staff on 6142 0970. Please try to call outside the clinic times, so we can deal with your call more efficiently.

Please call during the following times: Monday call after 12:30, Tuesday call before 13:00, Thursday call after 12:30, Friday call after 12:30. Please do not call for new patient appointments on a Wednesday.

All new patient appointments are held at the Hollywood Rooms.

Wednesdays are very busy days and only review appointments for existing patients can be accommodated at Joondalup. Wednesdays are usually fully booked for the following 3 months.


We almost always run to time.

New patients must arrive 15 minutes prior to their appointment time to complete paperwork. This will ensure you will be seen at the appointed time and have a full consultation. Late arrival will mean your appointment will have to be shorter.


Please call as soon as possible to cancel appointments if you cannot attend. The clinics are very busy, your slot can be reallocated if you tell us you aren't coming. Patients who fail to attend appointments will be charged for the time wasted (this will not be able to be claimed from Medicare). We require at least 24 hours notice for review patients and 48 hours notice for new patients.



New Patients Check List
Please bring the following with you:
-
Original referral letter
- Medicare Card
- Private Health Fund Card or details
- List of medications you currently take
- List of past medical conditions, illnesses and operations
- Any relevant pathology results, x-rays and scans
- Cash for parking at Hollywood Medical Centre (currently $3 per hour)
- Credit card, EFTPOS card or cash for payment of account (payment is required at the time of consultation). A receipt will be issued to allow medicare rebates to be claimed.
